*The area labeled "Course Format" in this document has been directly taken from Professor C. Ruiz-Velasco's Classroom Management Plan at CSUF as I found his classroom management extremely enjoyable and successful, along with most students, while working through my undergraduate work at CSUF.
* The area labeled "Academic Dishonesty" in this document has been directly taken from CSUF'S REFERENCE FOR FACULTY ACADEMIC INTEGRITY & DISRUPTIVE BEHAVIOR. I take cheating seriously and do not tolerate it in any way, shape, or form.

The above index card demonstrates a sample index card that students need to fill out for me to keep on file in class. The student information index card includes an area for their class schedule because sometimes late call slips come in from the front office/ administration, and also, from experience, students lose and misplace important personal belongings such as phones, purses, jewelry permission slips, check and money orders, fundraising products, and sports equipment, that may be found by myself or other students. Although I am a strong believer in taking responsibility for yourself, I understand students who may be forgetful, or have other needs. I am not having this assignment signed by parents/guardians as I need to make it clear to students that they need to be primarily responsible for themselves and begin the school year with trust from me that they turn in correct information (Though, they will know I have online access to their information as well).
